Updates for Fallout Shelter aren't on a strict schedule but tend to happen every few months or so. Bethesda rolls out patches, bug fixes, and sometimes adds new features or events, especially around holidays or to coincide with new Fallout game releases. It's been a bit more sporadic in the later years after its release, but they still keep the game fresh!

Every few months, usually. They've added new rooms, missions, and even pets over time. There's always something to look forward to!
