If you've somehow become one with a rock or a tree (happens to the best of us), try fast traveling to any location you've previously discovered. If fast travel's off the table because of enemies nearby, try jumping or sprinting in different directions to wiggle free. Also, toggling the collision off with the console command 'tcl' (PC only) works wonders - just remember to toggle it back on!
