Nah, he doesn't die. He gets super messed up after using the eighth gate against Madara, but he pulls through in the end.
Spoiler alert: Guy Sensei lives! Though he ends up in a wheelchair, he's still the legendary taijutsu master we all love.
He should've been dead, but plot armor saved him! Seriously though, he's alive but severely injured.
