They just use translators or English cards, simple as that. Most competitive players know what the cards do anyway.
As someone who's played Pokémon TCG at both local and international levels, I can tell you that language barriers are hardly an issue. Most players know the cards by their artwork and effects. In tournaments, you do need to have translations for your foreign cards, but everyone's there to have fun and celebrate Pokémon. So, language differences just add to the global camaraderie of the game.
