First, you gotta unlock the ballista by beating Yagluth and getting his drops. Then, use his drops to craft the Artisan Table. Once you've got that, gather 20 Iron nails, 10 Fine wood, and 10 Black metal, and you're good to go! Just place it like you would any other structure.
