First, sign into the same Microsoft account on both the app and the console. Next, in the app, go to 'My Library' -> 'Consoles' and select 'Connect to Your Xbox.' That should do the trick!

From my experience, all you need to do is make sure your Xbox and your phone are on the same network, sign into the same account, and the Xbox app should find your console automatically. Worked like a charm for me!
