Transfer the save file via USB or cloud storage like Google Drive from one computer to the other. Make sure you place the save in the correct directory on the new computer.
I've done this myself. Copy your save file from the original machine, usually found in C:/Users/[YourUsername]/AppData/Local/UNDERTALE, and paste it into the same folder on the new computer.
Just copy the save files from C:/Users/[YourUsername]/AppData/Local/UNDERTALE and paste them on the new PC. Simple as that!
