First, give the disc a gentle clean with a soft cloth, straight from the center to the edge. No circular motions! If that doesn't work, try installing the game to the hard drive. That way, the console reads the disc only for verification.

Make sure your console is horizontal, not vertical. Gravity is your friend! Also, check if the disc is scratched. If so, consider a professional disc repair service.

I had this issue and found that clearing the system cache helped. Go to System Settings -> Storage -> highlight your hard drive -> press Y and select 'Clear System Cache.' Worked like a charm for me!
