Easy peasy lemon squeezy! To unlock the 6th accessory slot in Terraria, all you gotta do is defeat the Wall of Flesh in Hardmode. Once you take down this big baddie in the underworld, it drops the Demon Heart, an item that when used, permanently gives you that much-coveted 6th accessory slot. Remember, this only works in Expert Mode or Master Mode worlds, so don’t sweat it if you can’t find it in a Normal world.

Actually, you can't get a 6th accessory slot in normal mode. You need to be playing in either Expert Mode or Master Mode. After you defeat the Wall of Flesh, it drops the Demon Heart. Use it, and boom, one extra slot for your accessories.
