I actually got this achievement by pure accident. Was fighting a horde of zombies with my health already low, and one of them landed a hit that brought me down to 1HP. I ran for my life, and bing! Achievement unlocked. Sometimes, it's all about being in the 'wrong' place at the right time.
Honestly, trying to get hurt in Terraria without dying is harder than it sounds. I'd say it's more luck than skill. Just play and it'll happen when you least expect it.
