Max health in Fallout Shelter boils down to a mix of proper Dweller placement and a sprinkle of strategic leveling up. From the get-go, give your Dwellers outfits that boost their Endurance stat, because more Endurance equals more health when leveling up. And here's the kicker - Dwellers gain health each time they level up, and the increase is based on their Endurance at that moment. So, if you equip them with high Endurance gear before they level, they'll get even more health. Also, keep those training rooms busy to max out their Endurance as early as possible. The earlier you start, the beefier your Dwellers will be.

Simple: Equip Endurance gear before leveling up. Max Endurance = Max Health. Easy peasy!

Honestly, I just throw them into the wasteland with the best gear I've got and hope for the best. Worked for me so far. They come back stronger... or not at all.
