Under launch options, type in '-fullscreen'. This makes it start up in fullscreen automatically.

Go to Options > Video > Display Mode, and select Fullscreen. Boom! Fullscreen goodness.

Easy peasy: Options > Video > Advanced > change the setting to Fullscreen. Kinda obvious when you think about it.
