To reach Home, you need to defeat Mother in Corpse II and then proceed to the Ascension sequence. After that, climb up several floors until you reach the final Home floor!

First, beat Mother in Corpse II. Then, follow the light beams during the Ascension sequence. Finally, go through a few more floors till you hit Home. Piece of cake!
