I've done it by beating Mom's Heart and getting taken down to Sheol or the Cathedral. Keep an eye out for special doors after the battle!
You can achieve a loop by defeating the final boss and choosing not to ascend, allowing you to go back to an earlier floor to replay.
Just defeat Mom鈥檚 Heart 11 times. Seriously, that unlocks the It Lives fight, which is key to looping.
