I use the Xbox Game DVR feature. Just press the Xbox button, go to 'Recording options,' and choose 'Record from now' or 'Record last' to capture gameplay.

You need an external capture card like the Elgato HD60 if you want to record longer clips or stream. Plug it into your PC and follow the setup instructions.
