From experience, you have to repeatedly choose 'Spare' in the ACT menu even if it seems like it's not working at first. Stick with it!

To spare Toriel, you need to keep selecting 'Spare' in the act menu during the fight. Just keep doing it, and eventually, she'll relent. You gotta be persistent!
