Oh, ready to face the squishy horror, eh? To summon the Brain of Cthulhu in Terraria, you need to break three Crimson Hearts found in the Crimson biomes' chasms. But proceed with caution! You can break them using a hammer or explosives. After you smash the third heart, the Brain will be raring to challenge you. Remember, this only works in a Crimson biome, so no luck in a Corruption world unless you create an artificial Crimson biome!

Just smash 3 Crimson Hearts in the Crimson area. Brain shows up after the third is broken. Bombs or a hammer will do the trick. Good luck!

Actually, you can also summon it with a Bloody Spine which you can craft at a Crimson altar with Vertebrae and Vicious Powder. Helpful if you're not into smashing hearts or need to fight it multiple times for those juicy drops.
