Back in the day, I unlocked it by holding both Block buttons and pressing U,L,A,R,D during the boot sequence. Easy peasy!

Just press U,L,A,R,D while holding the Block buttons when starting up the game.

To unlock the EJB menu in the original Mortal Kombat arcade, hold down both P1 and P2 Block buttons while pressing Up, Left, A, Right, Down (U,L,A,R,D) on the joystick when the game is booting. Voila, secret menu unlocked!
