Go to 'Profile & system' in the Xbox guide, select 'My profile,' then 'Customize profile.' From there, upload your custom image by selecting 'Change gamerpic' and 'Upload a custom image' from a connected USB drive.

Seriously? Just use the Xbox app on your phone, go to your profile, and upload the image from there.
