Not necessarily, but it's usually a good idea to toss the whole loaf. Mold can send out tiny filaments that spread throughout the bread, even if you can't see them. Plus, if one piece got moldy, the rest might not be far behind. Better safe than sorry!

Just cut off the moldy part and eat the rest. Bread's too good to waste.
If it's just one piece at the end, I sometimes just toss that piece and toast the rest. Heat kills some types of mold spores, but honestly, if you're not sure or if it smells funky, throwing it out is the safest bet.
