Yes, Mahito is indeed dead. As of the Shibuya Incident arc, Yuji and Todo gave him a serious beatdown. Then, Mahito's own ideology and actions led to his demise at the hands of Geto (who is actually being possessed by Kenjaku). It was a pretty intense and pivotal moment in the series!
