I'd say the rarest items are the Blue Dog and Pink Cat from the early pet updates. They don't come around often anymore.
The rarest items in Adopt Me are the Neon and Mega Neon pets, especially the legendary ones like the Shadow Dragon and Bat Dragon. Good luck getting your hands on those!
