Radiation in Fallout Shelter mainly comes from failing to manage your water supply properly. If you don't have enough water or your water treatment facilities aren't up to snuff, your vault dwellers will start soaking up radiation. Also, sending dwellers out to the wasteland without proper preparation can expose them to radiation. And don't forget those pesky radroach infestations; they're not just a nuisance, they're radioactive!

As someone who's spent more hours than I'd like to admit managing my own vault, radiation usually hits me when I least expect it. Water supply issues are the main culprit, but those radroaches and ill-equipped wasteland explorers contribute their fair share to my dwellers' glowing personalities.
