It usually means that not all members of your squad are ready to start the game. Sometimes it's because a friend is still in a match or hasn't hit the 'ready' button. A quick fix is to leave the party and rejoin. Works like a charm most of the time!

Honestly, it's often just a bug. Happens to me all the time. I just restart the game or wait a bit, and it sorts itself out. Annoying, but not the end of the world.
