Easy answer: She's part Tarkatan. Those nasty teeth come from the Tarkatan blood mixed into her DNA. Freaky, right?

Mileena's teeth are a result of her being a grotesque hybrid of Kitana and a Tarkatan, which gives her those gnarly chompers. Thank Shang Tsung's mad science for that!
