MLB The Show doesn't have tattoos because of licensing agreements and the complexities of accurately depicting unique player tattoos in the game. Tattoos can be protected as original works, meaning the game developers would need to get specific permissions from the tattoo artists and players for each tattoo. It's a legal and logistical nightmare!
From my experience, the reason they don't have tattoos is that it'd take a lot of extra work to get each one approved and designed. Just imagine trying to recreate every player's ink accurately!
