Make sure the code isn't expired or already used. You can usually find this info on the card or receipt.
Check if your region is supported. Some codes are region-locked and won't work outside their intended area.
Try redeeming the code directly on the Xbox website instead of the console. It sometimes works better.
I had this problem once, and it turned out the code was already redeemed. Contacting Xbox support solved it for me.
