Check if the disc is scratched or damaged. Sometimes minor scratches can make it unreadable.

Try cleaning the disc and the console鈥檚 disc drive. Dust and smudges can mess up the reading!

I've had this issue, and a quick reboot of the console fixed it. Seems basic, but sometimes that's all it takes.
