Nah, it's not like you'll die instantly if you don't, but it sure makes life in the wasteland tougher. Your character gets all sluggish and weak, which is a total buzzkill when you're trying to fight off a horde of super mutants. So, while you won't drop dead on the spot, neglecting your basic needs is a one-way ticket to Strugglesville.
