Nope, you don't need to pay monthly to enjoy Fallout 76. You just buy the game and you're good to go! There's no subscription fee for the base game itself, but there are optional in-game purchases available if you want to enhance your experience.

Definitely not! Once you purchase Fallout 76, it's yours to explore without any monthly fees. Bethesda does offer a subscription service called Fallout 1st which gives members some nice perks, but it's totally optional and not required to play the game.

Short answer, no. But, if you're looking to get the most out of your experience, consider the optional Fallout 1st subscription. It's not necessary but adds a lot of value.
