Nope, mutations in Fallout 76 don't remove or replace your perk cards. Think of mutations as a bonus (or sometimes a fun penalty) to your stats or abilities, completely separate from the perk card system. You can have a whole bunch of mutations and still keep all your perk cards in play.

Mutations won't mess with your perk cards at all! They add effects, both positive and negative, but your perk cards stay safe and unchanged. Kind of like adding extra toppings to your pizza; the base remains the same.
