Hot take: It aligns somewhat, but don't expect a 1:1 match. Fallout 76's version of WV is more 'inspired by' than 'directly copied from'. They squished some places closer together for gameplay purposes.
From firsthand experience, trekking through the wasteland of Fallout 76 feels like a distorted mirror of West Virginia. I've visited WV before, and finding places in-game that kinda match the real locations was an eerie experience. Not exact, but close enough to give me d茅j脿 vu!
