First, you'll need to log into your router's settings. Look for the Port Forwarding section and forward these ports: TCP: 3074, UDP: 3074, 3478-3480. Then, restart your PS4 and router. Voila, you should see an Open NAT type!
I had this issue last year. Turned out I needed to enable UPnP (Universal Plug and Play) in my router settings. Worked like a charm after that!
