Combining perk cards in Fallout 76 is simple! All you gotta do is level up to earn Perk Cards, and then when you have duplicates of the same card, you can combine them to make a stronger version. Just select the card you want to upgrade in your Pip-Boy, and if you have enough duplicates, there'll be an option to combine them.
