Super easy! Just go to Settings > Storage > System Storage > Applications. Find the game with the DLC, press Options, select 'Delete,' and confirm. Done!

From the main menu, navigate to Library, select the game with the DLC, press Options, go to 'Manage Game Content,' and delete the specific DLC you don't want.

I've had to do this a few times. Go to the PS4 home screen, then to the Library, select the game, and press Options. Choose 'Manage Game Content,' and you can select and delete the DLC you don't need.
