I had the same issue! Changing the chat audio from 'Chat Audio' to 'All Audio' in the settings fixed it for me. You can find it under Settings > Devices > Audio Devices > Output to Headphones.
Make sure no one else in the room is using a loudspeaker near a mic. Sometimes, a second mic picks up the sound and creates an echo.
