How to Get a Disk Out of PS4
Hold down the eject button for about 10 seconds. If that doesn’t work, turn off the PS4, unplug all cables, and manually eject the disk using a screwdriver through the manual eject hole underneath the cover.

Unplug it, wait 30 seconds, plug it back in, and try ejecting again. Works like magic for me every time!
I had the same issue! Just hold the eject button on the front of the console for a few seconds. If that doesn’t help, you might need to power cycle your PS4.
Try to eject the disk from the system menu. Go to the main menu, select the game, and choose 'eject disk.' Sometimes it just needs a little nudge from the software.
