First, head over to the Adventurer NPC on your island (he's usually chilling around the town area). Talk to him and look for the option to teleport to Slime Island. Easy peasy!

To get to Slime Island in Roblox Islands, first, you need to unlock the Access Pass by completing some quests given by the adventurer NPC. Then just take the portal at the main hub!
