Simply put, you can't make a Sim sick in The Sims FreePlay. They only get dirty or tired.

You can't really make a Sim sick in The Sims FreePlay. They can get dirty or unhappy, but no flu here!

There's no feature to make Sims sick in The Sims FreePlay. Maybe try making them miss work or perform poorly for some drama?

I tried leaving my Sim without food and sleep for days, but nope, they don't get sick! Just super cranky. It's all good fun though!
