On the Pokemon GO main menu, click the Poké Ball icon, go to the Shop, then scroll to the bottom and enter your code in the 'Promos' section. Easy peasy!

Just open the app, hit the Poké Ball, go to the Shop, and scroll down to the 'Promos' section where you can enter your code!
