You can revert your PSN name change by going to the Settings menu on your PlayStation console, then select Account Management > Account Information > Profile > Online ID, and choose your previous ID from the list. Just keep in mind, you can only revert once for free!

Head to the PlayStation website, sign in, and navigate to your profile settings to change your online ID back. It's quick and easy.
