Easy peasy! On your PS4, head to Settings > Account Management > Link with Other Services > Spotify, then just select Unlink. Voilà!

Had to do this last week! Just go to Settings, pick Account Management, then choose Link with Other Services, find Spotify, and hit Unlink.

Just head to the PS4's Settings, choose Account Management, then Linked Services, and unlink Spotify from there. Done and dusted.
