Quick fix: Turn off the PS4, connect the controller via USB, and hold the PS button until it disconnects.

Unpairing it is overrated. Just use the cable!

Just hold the PS and Share buttons together until the light bar starts flashing, then go to the settings to forget it on your console.

Easy peasy! Go to Settings > Devices > Bluetooth Devices. Find your controller, press Options, and select Forget Device.
