I upgraded to Premium through the Spotify app itself. You just have to click on the Premium tab and follow the prompts. Simple!
Just open the Spotify app, tap 'Premium' at the bottom, and follow the instructions. Easy peasy!
Don't do it through the app, they charge more. Use the website instead.
