Your PS4 might be possessed! Just kidding, it’s actually a well-known issue with the rubber foot under the eject button. Try tightening the manual eject screw.

The PS4 spitting out discs could be an issue with the eject button being overly sensitive. Try disabling the auto-eject feature in the settings or doing a system update.

I had the same problem. I fixed it by standing my PS4 vertically. It seems to reduce the pressure on the eject mechanism.
