Ghouls in Fallout 4 might seem immortal due to their long lifespans, but they're not truly immortal. They can live for centuries thanks to their unique mutation, but they can still die from physical damage or if their bodies decay enough over time. So, if you're planning on befriending any ghouls, just remember they won't be around forever, but likely a whole lot longer than you.

Nope, ghouls aren't immortal. They just have really, REALLY good skincare routines.

Ghouls? Immortal? Not exactly. They live longer than you'd hope if you're on their bad side, thanks to the whole radiation deal, but they can be killed just fine. Take it from someone who's spent too much time in the wasteland; a well-placed shotgun blast doesn't care about your lifespan.
