To throw a grenade in Fallout 4, first, make sure you have a grenade equipped in your inventory. Then, simply hold down the melee attack button (that's R1 on PS4, RT on Xbox One, and Alt on PC) instead of just tapping it. Give it a good long press until you see your character preparing to throw, then release to launch your grenade at your target. Happy exploding!
