Firstly, make sure to update your graphics drivers; outdated drivers often cause stuttering. Also, try lowering the graphics settings in the game - sometimes the preset is too high for your system, causing performance issues. Don't forget to check for background applications that might be hogging resources!

A quick fix that worked for me was enabling VSync through the NVIDIA or AMD control panel, instead of in-game. It somehow smooths things out better.

Just cap your FPS to 60. Fallout 4 doesn't play nice with framerates higher than that. Trust me, it's a game changer.

I've found that playing in Windowed Mode (not full screen) can actually reduce stuttering. Sounds weird, but give it a try!

SSD. If Fallout 4 is on an HDD, move it to an SSD. The faster read times seriously minimize stuttering caused by loading new areas or assets.
