Hold 'Alt' or 'RB/R1' depending on your platform, but don't forget to equip your grenade first! And try not to blow yourself up, lol.

Actually, aiming is overrated. Just run in, throw stuff, and see what happens. Fallout 4 doesn't judge.

Easy peasy, lemon squeezy! To throw grenades or any other throwable weapons in Fallout 4, just hold down the 'Alt' key if you're on PC, or the 'RB' button on Xbox, or the 'R1' button on PlayStation. Make sure you've equipped the grenade first, though. Just tap the respective button/key to equip or switch between your throwables. Aim, hold, release, and watch the boom!
