To start 'Hole in the Wall' in Fallout 4, first you gotta join the Brotherhood of Steel. After you've made friends with them, head over to Vault 81. You'll need to donate either three Fusion Cores or pass a tough Charisma check to get inside. Once you're in, spend some time wandering around, and eventually, you'll hear about a sick kid. That’s your cue! The quest will kick off once you talk to Dr. Forsythe, so listen out for gossip and keep those ears open!

Honestly, just make sure you've got enough Fusion Cores to get into Vault 81. The rest sorts itself out as long as you're not rushing through.

U just need to hang around Vault 81 for a bit after getting in. Help out, chat with folks. The quest will pop up naturally as you explore and talk to the NPCs. It's more about spending time there than anything specific.
